If the consultation is for your minor child/adolescent, please use the parent/guardian consent and child assent form. If the minor child cannot sign, please sign the parent/guardian consent form. This form explains psychotherapy treatment in general, tele-psychotherapy as an interim measure, process & termination of the therapy, and your consent. This ensures clarity for a smooth consultation process.
